First-Timer:

Degree/Course Requirement: Doctor of Optometry (O.D.)

Documentary Requirements:

A. Doctor of Optometry (6-year curriculum)

1) Original and photocopy of the Philippine Statistics Authority (PSA) - issued Certificate of Live Birth;

2) Original and photocopy of the PSA - issued Marriage Certificate (For married female applicants);

3) Transcript of Records with scanned picture and Remarks "for Board Examination Purposes"; and

4) Certificate of Intership of ten (10) months signed by the Dean and Registrar with the school seal.

 

B. Special Certification Examination

1) Original and photocopy of the PSA - issued Certificate of Live Birth;

2) Original and photocopy of the PSA - issued Marriage Certificate (For married female applicants);

3) Original and certified true copy of Certificate of Registration as Optometrist;

4) Original or certified true copy of updated Professional Identification Card;

5) Certificate of completion of the special course in Pharmacology prescribed by the CHED issued by a duly accredited institution, public or private, offering the prescribed course; and

6) Certificate of completion of preceptorial hours from duly accredited institution, public or private, from a Diplomate in Ophthalmology and a DPA - licensed optometrist.

Repeaters:

A. Doctor of Optometry (6-year curriculum)

Certificate of Completion of Refresher Course from duly accredited institution, after three (3) failures on the examination.

 

B. Special Certification Examination

Certificate of Completion of Refresher Course from a recognized institution, after three (3) failures on the examination.
